Executive Summary

Introduction

This report is based on the construction site safety issues in Australia with the example of the case which is being reported in the selected company. This incident was happened at the Alcove Engineering and Construction Pty Ltd. The building project to be successful, thorough planning, evaluation, and supervisory authority over health and safety obligations are crucial. The worst-case situation is avoided on one of the sites thanks to a security procedures system, which also boosts staff morale by lowering accidents. A fast introduction to safety management in construction is provided in this study, together with all the instruments required to launch the safety programs for the Australian construction sector.

It is essential for construction projects to have thorough health and safety plans, evaluations, and oversight. Risks may be reduced and worker morale booster for construction firms by adopting and enforcing stringent security protocols and safety management systems.

According to Shooshtarian et al (2023), an effectively managing safety is a challenging undertaking. All worksite safety-related activities are included in a safety management system. Case Description

According to Navaratnam et al. (2022), the Australian construction sector is growing fast worldwide but also confronting major difficulties, which makes it tough. A Safety Bay construction company was fined $45,000 and had to pay $5000 for a worker who fell three metres through a camouflaged hole. Alcove Engineering & Construction Pty Ltd was fined on Tuesday in the Rockingham Magistrate's Court after pleading guilty to failing to provide a safe workplace for a non-employee.

In late 2018, Alcove Engineering was appointed as the principal construction contractor to renovate a Safety Bay home with a second floor, according to Sutterby et al. (2023). A carpentry company and a glass company supplied and installed six double-glazed windows in the upstairs addition as subcontractors. Alcove Engineering and a carpenter subcontractor visited the site in January 2019 to prepare the top mezzanine floor for stairs.

After completing the installation, the carpenter assured the property owner that he would install an edge protection railing the following morning before glass business personnel arrived. Alcove Engineering's carpenter and site supervisor addressed the property owner's intention to cover the aperture with a tarp to keep wind and rain out of the lower level.

The carpenter removed the ladder to the top level and notified the site supervisor that no more workers would be at the property until 8:30am to build the railing. The worker agreed to return at 7am. Three glass firm personnel installed window panes the next morning between 7:00 and 7:30. The scaffolding took them upward. After raising the glass with the outside barrow ramp, an employee observed the tarpaulin on the floor in the upper living room. No barrier, edge protection, or signpost indicated an entrance below.

As he reached a living room window, the worker dropped three metres to the concrete floor below. Employee's shoulder and elbow were badly injured. Carpenter came shortly after the fall. WorkSafe Commissioner Darren Kavanagh was upset by another serious construction site fall accident. Mr. Kavanagh said that 14% of construction sector deaths during 2012/13 were caused by falls from height.

After an initial period of adjustment, the criteria for safety in high-risk construction projects will be increased due to WHS law. A Workplace Falls Prevention Code of Practise has been in effect in the Australian state of Western Australia from the year 2004. It is recommended by Tortorella et al. (2023) that this rule be followed in all jobs where there is a risk of falling. Construction businesses and employees should be familiar with the regulation's requirements since it has been in effect for more than twenty years. Chan and Aghimien (2022) contend that it was inappropriate for the site supervisor at Alcove Engineering to provide consent for the void to be covered with a tarp without first providing any warnings. According to the Safe Work Australia (2021) report, the most common causes of severe claims in the construction industry are Body stress (34%), Falls, trips, and slips (21%), Being hit by moving objects (14%) and Mental stress (9%). There are a disproportionate number of accidents and injuries in the construction business. With 3.3 serious claims per million hours worked, the construction industry is among the most dangerous in the economy (O'Neill et al., 2022). Falls, being struck by moving equipment, and exposure to toxic chemicals are the most common causes of injury on construction sites.

The carpenter had expected to see no workers at the site until 8:30 a.m. Due to the worker's unscheduled arrival, there was no prior warning or indication to exercise caution around the hole. The accident was caused by the carpenter's and worker's failure to coordinate their efforts and communicate effectively, as well as the lack of proper safety measures.

According to Sutterby et al. (2023) it seems that no proper safeguards were implemented to stop people from plunging into the abyss below. The railing on the upper mezzanine level wasn't installed right away since the carpenter in charge of the job waited until the next morning, when the essential supplies would finally arrive, to finish the job. This delay, however, left the gap unguarded and exposed, creating a serious safety risk. That's why, at the time of the incident, there were not any safeguards in place to stop people from plunging through the opening.

Project managers ensure all tasks are executed successfully, on schedule, and within budget. Contractors also must guarantee that their work complies with all applicable health, safety, and environmental standards.

On the building site, there were no major precautions taken. Failure to adopt required safety measures is shown by the lack of a railing, edge protection, or signs notifying the location of the hole. There seems to be a lack of thorough safety procedures at the site, since the installation of necessary safety equipment was delayed, and employees were not given enough warnings or precautions (Tsoi, 2020). The accident and the worker's injuries were exacerbated by the management's inability to properly prioritise and implement safety measures.

There is no proof that the employee had access to the essential tools. However, the statement that Alcove Engineering "failed to take any practical measures to ensure the safety of the workers" on the site suggests that they may have neglected to provide the necessary safety equipment to the injured party (Haufler, 2018). Accident victims may have been more severely hurt if they hadn't been without basic safety gear.

Stakeholders' regular checks of the site's safety are not mentioned. To maintain the industry's safe and sustainable operations, this demands constant dialogue and cooperation.

Case Analysis and Recommendation

Root Cause of Accident

According to the Occupational Safety and Health Administration (2022), claims that the falling of employees during the construction has becoming very common in Australia. The root cause of accidents is the mis management of Alcove Engineering and Construction Pty Ltd. which is not providing the proper facilitation to employees regarding the training and resources. Many of these mishaps are the consequence of carelessness or negligence of any employee. These could have been avoided as a result. Those who get injured in these accidents must pay expensive medical expenses and miss work, which results in lost wages. The provision of fall-prevention equipment by contractors is necessary. Companies must also construct and maintain perimeter security, lock up floor entrances, and employ ladders and scaffolds that have OSHA approval. Falls that result from inadequate equipment can result in severe injuries or even death.

Responsibilities of Stakeholders

According to Desk (2020), the responsibilities of stakeholders regarding the construction site is to lend their expertise and point of view to a certain project to make the organization meet up their desired goals and objectives. On the other hand, stakeholders offer numerous resources and materials which are required by the organization and its employees to fulfil the needs. For the above selected case of Alcove Engineering and Construction Pty Ltd. where several employees died due to mis-management, the duties of stakeholder are to follow up frequently to ensure that their construction project is not negatively impacting society. Many stakeholders act to start projects that will benefit their local society, going beyond just making sure there is no harm to society. Stakeholders for construction industry are known as the building experts who are assembled for the entire project and then get divided when the project ends successfully.

Safety Management Systems

According to Agus Salim et al. (2023) claims that, the main objective of safety management system is to provide the systematic method to an organization for handling the operations safely and effectively. The SMS can provide the Alcove Engineering and Construction Pty Ltd. the guidelines and procedures through which it can ensure the safety of lives of its employees. By making the advancement in the SMS procedure exemplifying corporate diligence, and fostering a culture of overall safety. This method and technique provide to increase safety. Particularly in high-risk industries like aviation, electricity, shipping and construction, effective safety management is vital to preserving procedures and growing the firm.

To ensure the safety of employees and lower the risk of accidents and death the Safety management system or SMS are beneficial for Alcove Engineering and Construction Pty Ltd. It helps the organisation to face the challenges effectively and efficiently. One way to think of it is a fusion of risk analysis and quantity assurance ideas. SMSs involves around the planning, organizing, and deploying with your organisational structure in mind, much like other management tasks. Its safety procedure includes the hazards detection, reporting of risk occurrence, risk management, performance evaluation and quality-based safety precautions for employees.

Recommendations

The study makes various suggestions to improve Australian construction site safety. Safety protocols, the use of equipment, and the recognition of potential dangers should all be top priorities for Alcove Engineering and Construction Pty Ltd. It's important to make sure people have easy access to information and safety equipment like harnesses and helmets in the case of a fall. Construction and maintenance of perimeter security, locking of floor entrances, and the use of OSHA-approved ladders and scaffolds are all essential for ensuring the safety of workers. It's important to do regular safety checks. Constant communication with community members throughout building is essential for minimising disruptions and maximising benefits to the area. Decisions should be made with stakeholder input, knowledge, and resources. It is advised that a thorough safety management system be established, one that accounts for the identification of hazards, the mitigation of risks, the assessment of results, and the promotion of ongoing enhancements. The efficiency of the system should be regularly audited and assessed. Create a safe workplace by rewarding good behaviour, recognising and discussing any safety concerns, and opening up lines of communication. To increase worker awareness, provide training and meetings on a regular basis.

SMS offers a structured method for managing activities safely and efficiently, fostering an environment where safety is prioritised above all else. Precautions are taken to ensure quality while also identifying, reporting, managing, and evaluating any risks. Companies like Alcove Engineering may improve safety procedures, reduce hazards, and safeguard personnel by using SMS.
